Supported and Community Living

Who we are:

  • Sunrise Services offers a continuum of community living services for adults with a wide variety of developmental disabilities and levels of skill and independence. We gladly accept participants who have major behavioral challenges, as well as those who have a dual diagnosis of developmental disabilities and mental illness or traumatic brain injury.

What we do:

  • We help individuals: develop self-determination, independence and competence by providing needed supports for daily living by providing services that are focused around goals identified through a person-centered planning process between you and your family.
  • We help develop a unique, individual vision for the future, and assist you in creating a plan with clear, meaningful, steps to achieve this vision. Family, friends, and community connections are gladly welcomed and included as determined by you.
  • Participants live semi-independently in their own homes – houses or apartments – and have the opportunity to select their own menus, do their own cooking, and make decorative choices about their homes. Participants are highly encouraged to select staff with which they feel most comfortable.

How do I get started with Sunrise Supported and Community living services?

  • Referrals for adults with developmental disabilities are routed through Department of Social and Human Services (DSHS) and the Division of Developmental Disabilities (DDD). Contact DDD Case Manager and request that a referral be made to Sunrise Services.
  • Referrals for adults with mental illness or traumatic brain injury are routed through DSHS and HCS. Contact your HCS Case Manager and request that referral be made to Sunrise Services.

How much support can I get?

  • Staff-to-participant ratios are designed to provide optimum support and assistance. The number of hours of service received from Sunrise Services staff and the type of assistance given are based on participant needs. Staff train and support participants in the full spectrum of daily living tasks.
